Service Options Using Resources in a Community Environment (SOURCE)

The Service Options Using Resources in a Community Environment (SOURCE) program is a Georgia Medicaid program that connects primary medical care and case management with long-term services and supports delivered in the home or community.

SOURCE is designed for individuals who are eligible for full Medicaid, meet nursing home level of care, and can be safely supported outside of an institutional setting.

What SOURCE Provides

SOURCE combines medical oversight and case management with community-based long-term care services to help eligible individuals remain safely at home or in another community setting.

Depending on the person’s needs and approved care plan, services may include:

  • case management

  • personal support services

  • assisted living services

  • extended home health

  • home-delivered meals

  • adult day health care

  • emergency response services

  • coordinated access to primary care and community-based supports

SOURCE is built to support both medical monitoring and assistance with daily living needs.

 

Who SOURCE Is For

SOURCE is generally intended for individuals who:

  • are eligible for full Medicaid

  • meet nursing home level of care

  • are frail older adults or physically disabled adults

  • need both medical oversight and long-term support services

  • can be safely served in a home or community-based setting

A comprehensive assessment is used to determine eligibility and identify the level of care and support needed.

 

How Care Is Planned

Each SOURCE participant receives an individualized care plan based on assessed needs.

 

The care plan may consider:

  • medical monitoring needs

  • help with daily living activities

  • safety and support concerns

  • current living situation

  • available family or caregiver support

  • community-based service options
