Independent Care Waiver Program

The Independent Care Waiver Program (ICWP) helps eligible adults with severe physical disabilities or traumatic brain injuries receive support in their homes or community settings instead of in a hospital or nursing facility.

 

This Georgia Medicaid waiver program is designed for adults who need ongoing assistance but want to remain as independent as possible in the community.

 

At 360 Cares, we help families understand the program, navigate the process, and coordinate support services once eligibility is established.

Who ICWP Is For

ICWP is generally intended for Georgia Medicaid members who:

  • are between the ages of 21 and 64 at the time of application and when services begin

  • have a severe physical disability and/or traumatic brain injury

  • need assistance with one or more activities of daily living

  • are medically stable, but at risk of hospital or nursing facility placement without community-based support

  • can be safely served in a home or community setting

For individuals with traumatic brain injury, some eligibility rules may differ.

 

Services Available Through ICWP

Depending on eligibility and approved care needs, services may include:

  • case management

  • personal support services

  • adult day health

  • home care services

  • respite care

  • emergency response services

  • specialized medical equipment and supplies

  • counseling

  • home modifications

For individuals who qualify, alternative living services may also be available in a residential setting.

 

How Services Are Planned

Each participant’s services are based on an individualized plan of care.

This plan is developed through collaboration between the individual, family or support system, case manager, and service providers. The plan looks at:

  • current living situation

  • strengths and support needs

  • daily care needs

  • goals and preferences

  • available services and providers

  • projected cost of care

Approval depends on both eligibility and the availability of waiver funding.

 
Important Things to Know

Meeting basic eligibility requirements does not automatically guarantee immediate services. Additional factors may affect when services become available, including:

  • whether the individual is currently in a hospital or nursing facility

  • time spent on the waiting list

  • ability to live safely in the community

  • whether community-based care can be provided at a cost that does not exceed institutional care

    Georgia Medicaid waiver programs are designed to offer home- and community-based services as an alternative to institutional placement when appropriate.
