Community Care Services Program

The Community Care Services Program (CCSP) is a Georgia Medicaid home- and community-based waiver program that helps eligible individuals receive support services in the home or community as an alternative to nursing home placement.

CCSP is designed for individuals who need a nursing home level of care but prefer to remain in a home or community setting when it can be done safely and appropriately.

What CCSP May Provide

Eligible individuals may receive a combination of Medicaid-funded CCSP services and other community-based supports based on their needs and approved plan of care.
Services may include:

  • Adult Day Health

    a medically supervised day program that may include nursing oversight and therapy services

  • Alternative Living Services

    personal care home placement with 24-hour supervision for individuals who qualify

  • Emergency Response Services

    in-home emergency alert systems for added safety

  • Home-delivered Meals

    nutritious meals delivered to the home

  • Home-delivered Services

    skilled nursing and therapy services in the home when appropriate

  • Out-of-Home Respite Care

    temporary relief for family caregivers

  • Personal Support Services

    help with personal care, meal preparation, light housekeeping, shopping, and in-home respite

For some individuals, structured family caregiver support may also be available based on eligibility.

Who May Qualify for CCSP

CCSP is intended for individuals who meet certain functional, medical, and financial requirements.

In general, a person may qualify if they:

  • have functional limitations related to a physical condition

  • require an intermediate level of nursing home care

  • have physician approval for needed care

  • are Medicaid eligible, or potentially eligible after financial screening

  • choose to receive services in the home or community rather than in a nursing home

  • participate in only one waiver program at a time

Alzheimer’s disease and other forms of dementia may also be considered qualifying physical conditions for purposes of the program.

Steps for CCSP Eligibility Screening

Step 1: Contact the Area Agency on Aging

To request screening for CCSP services, individuals typically contact their local Area Agency on Aging (AAA). The toll-free, statewide phone number is 866-55-AGING (866-552-4464).

Step 3: In-Home Assessment

When services become available, the assigned care coordination agency will contact the individual to schedule an in-home assessment. This assessment helps determine the level of care needed and supports development of the care plan.

Step 2: Screening and Waiting List

If screening shows that the individual may meet nursing home level of care and financial eligibility requirements, the person may be placed on a waiting list based on level of need.

Step 4: Care Plan and Service Start

Once eligibility and level of care are confirmed, services can be arranged based on the approved care plan.

Possibly. Individuals with limited income and assets may still be screened for potential eligibility. Final Medicaid financial eligibility determinations are made through the appropriate state process.
